Safe Routes to School Parent Survey

The Mattawan Consolidated Schools are participating in the Safe Routes to School Program.  Safe Routes to School is a national program to make it safe, convenient and fun for kids to bicycle and walk to school.  These questions will help us learn your views about walking and biking to school.  This survey will take 5 minutes to complete.  We ask that each household complete one parent survey.  You indicate your voluntary agreement to participate by completing and submitting this survey.  However, if you choose not to complete all or part of the questions, you or your children will not suffer any penalty.

Your responses will be kept confidential and your name will not be associated with any results (you do not have to put your name on this survey).  Thank you for participating.


The first set of questions ask about how your children travel to school.  If your children live in more than one household part of the time, please answer these questions for the home at which they spend most of their school days.

The second set of questions ask your thoughts about walking/biking to school.


 

Example: if your child walks to and from school everyday you would put 10

Please check for each grade that you did walk to school

If you select yes skip the next question, if you select no please tell us why in the next question

Example if you feel strongly about the positive health benefits your child(ren) get walking to school, please choose very box.  If you feel that walking to school is somewhat unhealthy choose the moderately box.
